Before you file your documents at the court registry, make photocopies for your records. You need two copies of the:

• Requisition for Consent Order or for Order Without Notice (Form F29)
• Affidavit (Form F30)
• Draft Consent Order (Form F33)
• Agreement as to Annual Income (Form F9)
