Fill out the Financial Statement

You'll need:

The information you need to put into your Financial Statement will be in the documents that you need to be attached to it (see Step 2).

The Financial Statement contains instructions to help you fill it out. You can either fill it out online or print it out and fill it out by hand (print neatly using dark-coloured ink).

The Financial Statement is divided into six parts. You may not have to fill them all out. The form itself will tell you which parts to complete.

  • Part 1 — Income
  • Part 2 — Monthly expenses
  • Part 3 — Property
  • Part 4 — Special or extraordinary expenses
  • Part 5 — Undue hardship
  • Part 6 — Income of other persons in household
