45: Affidavit — General (Provincial Court)
Use this form to create a document that states facts that you want to present to the court when you apply for an order (for example, for support) and that you swear under oath or affirm to be true. An affidavit should be divided into numbered paragraphs and not be longer than 25 pages, including exhibits, unless you have permission from the court to have a longer affidavit.
