45: Affidavit — General (Provincial Court)

Use this form to create a document that states facts that you want to present to the court when you apply for an order (for example, for support) and that you swear under oath or affirm to be true. A lawyer, notary public, or commissioner for taking affidavits usually needs to witness your signature, but you might be able to file it with the court unsworn.
